Join us for a workshop to empower independent pharmacies in advancing chronic disease prevention and management.
Experts will highlight evidence-based programs that expand access to care, including the Diabetes Prevention Program (DPP), Diabetes Self-Management Education and Support (DSMES), and Heart Healthy Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) Program.
Participants will gain practical strategies, workflows, resources, and connections to successfully implement these services, strengthen patient engagement, and improve health outcomes across their communities.

Activity Title:
Empowering Independent Pharmacies to Improve Health Outcomes: Implementing Disease State Management Programs through Community Partnerships

Learning Objectives:
At the completion of this workshop, pharmacists will be able to:
- Differentiate between the Diabetes Prevention Program (DPP), Diabetes Self-Management Education and Support (DSMES), and Home and Healthy Heart Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) Programs.
- Apply strategies for patient engagement, referral systems, and billing preparedness within independent pharmacy practice.
- Examine opportunities for sustainable integration of chronic disease management programs in community settings.
- Produce an action plan to incorporate at least one evidence-based program into their pharmacy practice.

This event has been approved for 3 contact hours (0.30 CEU). Participants must sign in and attend the entire session. No partial credit will be issued. Participants will complete the program evaluation online within 15 days for transmission to the CPE ® Monitor System.
